About the service
The injury to the brain caused by a stroke can lead to widespread and long-lasting problems.
Although some people may recover quite quickly, many people who have a stroke will need long-term support to help them manage any difficulties they have and regain as much independence as possible.
This process of rehabilitation will be specific to you, and will depend on your symptoms and their severity.
We provide as much information and support on the effects of stroke as we can to patients, their family and their carers.
We work with other community services to make sure care packages and rehabilitation programmes are in place, and ensure regular post-stroke reviews take place with the appropriate healthcare professional.
Part of our work is to raise awareness of stroke.
Who is this service for?
For adults diagnosed with Transient Isceamic Attack (TIA) or had a stroke confirmed by CT/MRI scan. We will work with patients up to two years post-stroke.
