About the service

The aim of the Residential Home Support Team is to reduce hospital admissions and upskill care home staff.

The service provides training for care homes on 14 different subjects:

- Catheter Care and Urinary Tract Infections (UTI)
- Constipation
- Delirium and challenging behaviour
- Dementia awareness
- Diabetes awareness
- End of life
- Falls
- Frailty
- Malnutrition Universal Screening Tool (MUST)
- Nutrition and hydration
- Oral health
- Pressure ulcers and waterlow
- Sepsis
- Skin care.

The service also specialises in:

- Comprehensive geriatric assessment
- Implementing new ways to prevent hospital admission and speed up discharge
- Ordering equipment if required
- Onward referral to other services if required
- Supporting care home to be compliant with standards
- Supporting care homes to transfer patients if required
- Supporting care homes with complex patients
- Monthly managers meetings
- Supporting care homes with patients at the end of life.
