About the service

NHS Continuing Healthcare (CHC) is a package of care arranged for people outside of hospital who have ongoing healthcare needs. You can receive continuing healthcare in any setting, including at your own home or care home.

NHS continuing healthcare is free, unlike support provided by local authorities where a financial charge may be made depending on a person’s income and savings.

NHS-funded Nursing Care (FNC) is for people living in care homes who have recognised nursing needs. These care homes employ registered nurses to meet the identified nursing needs.

People should receive NHS-funded nursing care when assessed as requiring the services of a registered nurse; the NHS makes this payment direct to the care home. 

We are commissioned by North Somerset Clinical Commissioning Group to assess eligibility for funded care compliant with the National Framework criteria for CHC and FNC. This service is delivered by the Funded Healthcare Team.
