Clevedon Community Hospital
Clevedon Community Hospital delivers rehabilitation care and the prevention of acute hospital admission for people from North Somerset. We also have:
- A Minor Injury Unit
- An outpatient department
- X-ray (radiology) unit (run by North Bristol NHS Trust)
Clevedon Community Hospital offers care within two ward areas for non-acute patients requiring nursing, medical attention and therapy to recover from an episode of illness or a medical intervention.
Medical cover is provided by doctors from the Clevedon GP Practices who visit regularly throughout the weekdays. During evenings, overnight, bank holidays and weekends we access medical care through the North Somerset GP Out Of Hours service. There is also access to specialist consultant advice where necessary.
Our healthcare professionals such as nurses, doctors, physiotherapists and occupational therapists work together as a multi-disciplinary team with our patients and their carers to deliver care that is designed to meet individual needs and ensure patients recover and are able to care for themselves as much as possible in preparation for leaving our care. If appropriate, some of this care will also continue after patients have been discharged.Back to top
Patient rehabilitation at Clevedon Community Hospital
It is expected that our in-patients will work towards an expected discharge date. The activities during the day will reflect this, and individuals will be encouraged to take an active part in their rehabilitation.
All activities will be established following assessment by a therapist. The patient will then be able to participate with the help of a Generic Support Worker who has been assigned to them.
Rehabilitation will take place both on the ward and in the dedicated therapy rehabilitation area (gym). Outdoor activities will also take place if appropriate.Back to top
Who is this service for?
We treat adult patients of all ages in our inpatient unit and outpatients department.
In our Minor Injury Unit, we also treat children above the age of 3 years.Back to top
How to access this service
Patients can access the hospital (inpatient admission, outpatient appointment, x-ray appointment) by being referred through their GP or through a clinician from an acute trust.
For minor injuries, there is a walk in service – you do not need to be referred by a doctor.Back to top
